Your medical data

Following the latest news around patients' medical data and NHS Digital's plans to extract this under the new data collection proposal called General Practice Data for Planning and Research (GPDPR), we wanted to make you aware of this change that is coming and share some of the resources that are available to help you decide and be more informed about this collection and what it will mean for you.

Please note that earlier this week it was confirmed that they are delaying the first collection of data and this will now happen on 1 September 2021. Patients who want to opt out will need to do so before this date allowing for time for the form to be processed, currently we are advised that patients need to complete this Type 1 form and return it to the surgery for us to code on your medical records.

To opt out of the GPDPR data collection, please print and complete this Type 1 form and return to the surgery by hand, in the post or by scanning and emailing to hwccg.haresfield@nhs.net. We will endeavour to process these forms within one working week.

As this has been instigated by NHS Digital, we are awaiting further information on how patients without internet or mobile phone access will be supported with being informed and making their decision. We hope that NHS England will write to patients directly, but if not then the practice will plan to do so as soon as we receive more information of their plans.
